I think when you're still moving and then your controller is not working / turned off / lost tracking at same time while pressed function is active.
This will cause avatar will still act on last pressed button. If you move sideways while controller went off, it will stay walk sideways.
It's better avatar stop and going in neutral when controller is lost.
