Website to Report Users/Players

We've been using the Moderation Channel on Discord for logging bad players and for emergencies, but I think we should have a more solid way of reporting players.
My idea is that we would still keep the Discord Channel for emergencies and logging stuff, but have some sort of website or even on the VRChat website itself a way of filling down a Template/Form for reporting players, it would REQUIRE us to be logged in to our VRChat account so it says who's reporting it and all that and we could have a simple form like:
- Reported User(s)/Player(s) Usernames(s)
- Time(s) & Date(s) (optional)
- Details/Info
- Proof (Media)

These reports would be into a Ticket kind of System so Moderators can handle them well and 'claim' them so they deal with it and go from there, they can archive the reports and have them ready for future references if needed, also possibility to filter reported names to see how many time someone got reported etc...

owlboy: Not sure, but outside of VR, there's some games where their communities use this sort of system to handle reports, I've been on some of them and on the Administration side and I found it quite organized and good to handle multiple and detailed reports that we could take our time dealing with them compared to emergencies or in-game real time moderation. I suggested it first directly to VR Pill cause I was not sure, he told me it was a good idea and should put it on canny so I did.
