Spatialized voice position doesn't follow head

In build 791, the apparent position of a user's voice when speaking fails to track their head position, instead it seems to come from a point a little beneath their nametag. I only experienced this bug from a user using FBT (and me also using FBT, they experienced the same bug with my voice). it may still work using 3-point tracking. Voice still follows in the XZ plane, just not vertically.
Steps to reproduce:
- Find somebody using FBT.
- Ask them to crouch or lay down, or use playspace mover to move vertically up or down.
Expected:
Their voice sounds like it comes from their avatar's head position.
Actual:
Their voice comes from their head position as if they were standing upright regardless of where their avatar's head is.

Technically a version of this still rarely happens but it's fixed with an avatar hide/show. Not the same cause though.
Either way that bug is being tracked separately elsewhere and this 2-year-old post references an issue that was fixed a while ago. Similar symptoms may appear elsewhere but the bug is either a regression or a different cause.

This should be mostly solved now, but there are some cases we're seeing where the voice audio source isn't moving properly. Its fairly rare now, though.

The voice now moves up/down with the head, but it isn't moving forward or to the side at all. If you lean forward, your voice will be coming from behind your head. Not fixed and not "rare", it's easy to reproduce this way.
